  • Form ATT-17 - Georgia Alcohol & Tobacco Personnel Statement (requires notarization)
  • Citizenship Affidavit along with secure and verifiable documents, i.e. government issued photo ID. (requires notarization)
  • Scanned copy of current annual registration from the GA Secretary of State
  • Scanned copy of Federal Basic Permit
  • Scanned copy of Local License for Ethanol Manufacturing
  • Power of Attorney (if applicable)
  • Financial Affidavit
  • Scanned copy of Federal Label Approvals

License Description

Non-Beverage Manufacturer/Importers - Non-Beverage Manufacturer/Importer means ethyl alcohol used; for scientific, chemical, mechanical, industrial, medicinal and culinary purposes; for use by those authorized to procure ethyl alcohol tax free, as provided by federal law; in the manufacture of denatured alcohol or denatured distilled spirits produced and used as provided by federal law; in the manufacture of patented, patent, proprietary, medicinal, pharmaceutical, antiseptic, toilet, scientific, chemical, mechanical and industrial preparations or products unfit for beverage purposes; or in the manufacture of flavoring extracts and syrups unfit for beverage purposes.

Non-Beverage Manufacturer/Importers License does not have a license fee.
